Because claimant failed to cooperate with the process. Clmt had been receiving benefits since 1992 (Title II & XVI)
SI (as of 2/1/2004): Schizophrenia; morbid obesity; herniated cervical disc; degenerative disc disease, carpal tunnel syndrome, drug abuse.
ALJ Found Medical Improvement as of 2/1/2004. Found that, based on claimant's testimony, claimant could perform Sedentary work.
11/30/2006: Complaint of pain and weakness in right wrist. On physical examination, decreased grip strength accompanied by atrophy of the wrist musculature and positive Tinel's and Phalen's signs. DX: chronic uncontrolled hand pain and enthesopathy of the wrist and carpus.

On physical exam wheezing was present
Severe pain in the lower lumbar spine radiating to right leg
DX: Sciatica, Baker's Cyst, and L. Knee degenerative Joint Disease. Prescribed Ibprofen and Percocet, and told to follow up with an orthopeadic surgeon.
C/O chronic back and leg pain. Impression: low back pain w/ sciatica. Tx: epidural Injection
Revealed DDD of cervical spine: at C3-4, broad-based herniation causing moderate effacement of the thecal sac and bilateral neural foraminal narrowing; at C4-5, disc buldge with hypetrophy of the unconvertable joint; at C5-6 and C6-7. broad-based osseous ridging and sic herniation and bilateral neural foraminal narrowing.
Revealed changes indicative of tendinosis along the distal one-third of the supraspinatus joint, and moderate DJD of the acromioclavicular joint with impingement.
mild degenerative changes. Soft tissue calcificaiton in the region of the carpal tunnel.
5th PIP arthritic changes
Moderate multi-compartment arthritic change
mild-moderate arthritic change involving the medial tarsal bones and the first MTP joint with bulky dorsal spurring; hallux valgus deformity and bunion formation
Revealed Baker's Cyst that had enlarged since 3/11, currently at 5cm.
Revealed Hypertrophic spondylitic degenerative changes of lumbar spine. Discogenic degenerative narrowing.
Revealed osteoarthitic degenerative changes with moderate to large left knee joint effusion.
Revealed prominent dorsal soft tissue swelling at the level of the metacarpal heads; mild degenerative change at the fifth proximal interphalangeal joint (stable since '09)
Limited to Sedentary work
